Service focus areas include food, housing and transportation, and toxic stress and interpersonal violence.\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Food services include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>SNAP enrollment assistance<\/li>\n\n\n\n<li>Group nutrition classes<\/li>\n\n\n\n<li>Diabetes prevention program<\/li>\n\n\n\n<li>Produce prescriptions<\/li>\n\n\n\n<li>Healthy food boxes and healthy meals (pick-up or home delivery)<\/li>\n\n\n\n<li>Medically tailored meals (home delivery)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>How they do it:<\/strong> Individuals who qualify for HOP services must be <a href=\"https:\/\/ncmedicaidplans.gov\/en\">NC Medicaid Managed Care<\/a> members living in one of the <a href=\"https:\/\/www.ncdhhs.gov\/healthy-opportunities-pilots-map-pdf\/download?attachment\">pilot region counties<\/a> and have at least one qualifying physical or behavioral health condition and one qualifying social risk factor (ie. homelessness, food insecurity, or lack of transportation).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Care managers work with Medicaid members and all Medicaid Prepaid Health Plans to identify individuals who qualify and would benefit from HOP services and to track and manage individuals\u2019 services over time.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Each region has been assigned a Network Lead organization &#8211; <a href=\"https:\/\/impacthealth.org\/\">Impact Health<\/a> manages the <a href=\"https:\/\/impacthealth.org\/healthy-opportunities-pilot\/\">Western NC region<\/a>, <a href=\"https:\/\/www.accesseast.org\/\">Access East<\/a> leads the <a href=\"https:\/\/www.hopaccesseast.org\/\">Eastern NC region<\/a>, and <a href=\"https:\/\/www.carelcf.org\/\">Community Care of the Lower Cape Fear<\/a> oversees the <a href=\"https:\/\/capefearhop.org\/\">Cape Fear region<\/a> &#8211; that serves as the connection point between the health care and social services sectors. Network Leads are responsible for contracting with human service organizations (HSOs) in their region to provide services for all eligible Medicaid members, including pregnant women, children, and adults. HSOs for food services include food banks and pantries, nonprofits supporting farmers, food producers and retailers, and healthcare providers.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Evaluation is a key component of the HOP program and is conducted by NCDHHS in collaboration with the <a href=\"https:\/\/www.unc.edu\/\">University of North Carolina<\/a>\u2019s <a href=\"https:\/\/www.shepscenter.unc.edu\/\">Cecil G. Sheps Center for Health Services Research<\/a>. The evaluation includes measuring the effectiveness of non-medical interventions and the role of the Network Lead to improve health outcomes and reduce healthcare costs.
