Inside the Human Healthspan Project: Making Prevention Predictable

Most of medicine still waits for something to go wrong before it acts. A new research effort led by Dr. Nathan Price at the Buck Institute for Research on Aging is built on the opposite premise: that the years before disease strikes are exactly the years we should be studying most closely. That effort — the Human Healthspan Project, anchored by a new initiative called Healthspan Horizons — aims to make healthspan itself measurable, predictable, and modifiable, using AI, systems biology, and precision nutrition (Buck Institute).

Why Healthspan, Not Just Lifespan

The project’s starting premise is simple but radical: adding years to life matters far less than adding healthy years. Dr. Price, Professor and Co-Director of the Buck Institute’s Center for Human Healthspan and Chief Scientific Officer of Thorne HealthTech, has argued that even eliminating every form of cancer would add only about two years to average lifespan — while a population that aged as well in its later decades as it does in its 20s and 30s would be transformative for both health and healthcare costs (Buck Institute interview). The Human Healthspan Project treats aging itself, not any single disease, as the target.

The Data Foundation: From Pioneer 100 to a Federated Model

The project builds on more than a decade of longitudinal research. It traces back to the Pioneer 100 Wellness Project, a 2014 study tracking over 100 participants’ genomics, proteomics, metabolomics, and microbiome data over nine months, and its commercial successor, Arivale, which followed nearly 5,000 people’s biology over five years — producing one of the largest longitudinal multi-omic datasets on human wellness ever assembled (Nature MedicineNational Academies).

Healthspan Horizons takes a different architectural approach to that legacy: rather than centralizing all data in one place, it’s built as a federated data intelligence platform, where, in Dr. Price’s words, “intelligence moves to us, but data stays where it is safely governed” (Healthspan Horizons demo). The goal is to let researchers, clinicians, and biotech partners tap into a much larger pool of health data without requiring everyone to give up control of it first.

The AI Behind the Project: Compass and Synthesizer

At the technical core of the Human Healthspan Project are two AI systems Dr. Price’s team is building. Healthspan Compass is aimed at the individual — an agentic AI system, including a component called the N-of-1 Analyzer, designed to interpret one person’s genomics, biomarkers, and lifestyle data and translate it into personalized insight. Healthspan Synthesizer works at the population level, automating the kind of discovery process that once took Dr. Price’s team years to produce as published research — with the aim of running that analysis “every night, basically, or many times a night” (Healthspan Horizons demo).

The project has also produced some of its most tangible results in digital twins — computer models of an individual’s biology that simulate how their health might unfold and which interventions could change that trajectory. Dr. Price’s team has applied this to Alzheimer’s risk, modeling how personalized combinations of interventions, rather than any single treatment, may meaningfully delay onset (BioSci Consulting).

Precision Nutrition as a Core Pillar

Nutrition sits at the center of the Human Healthspan Project’s approach to prevention. Research from the Arivale cohort found that a person’s genome can predict whether diet alone will meaningfully lower their LDL cholesterol, and that gut microbiome composition — not genetics — was the strongest predictor of who successfully lost weight over the course of the study (NutraIngredients). These findings underpin the project’s broader argument: that “eat this, not that” guidance needs to give way to recommendations built on each person’s own biology.

The Bigger Case: Scientific Wellness

The Human Healthspan Project is, in many ways, the next chapter of an argument Dr. Price first laid out with Dr. Leroy Hood in their 2023 book, The Age of Scientific Wellness — that data-rich, predictive, participatory medicine (so-called P4 medicine) should become the default model of 21st-century healthcare, not a premium add-on for the few who can afford it (PMC). Dr. Price has continued to push that case as a member of the National Academies’ Board on Life Sciences, focused on the responsible application of AI to extend healthspan (National Academies).
